We are still waiting on final approval of the new Form 4506-C which will be used specifically for the express verification program. The latest revision of Form 4506-T (Rev 6-2019) has line 5a removed and is now exclusive to the RAIVS program. Until the new Form 4506-C is available, continue using the March 2019 revision of Form 4506-T (Rev 3-2019).
